A licensed person or pharmacy
Keep a copy. Add the names and dates you know. Save each reply.
As of September 2026, a complaint about a licensed Indiana doctor, nurse, physician assistant, pharmacist, or pharmacy goes to the Attorney General’s Licensing Enforcement unit, not straight to the licensing board.

Privacy before filing
Read the privacy notes before filing. Decide what details you can share, and if you go through the licensing agency, ask it how your name will be handled.
As of September 2026, Indiana Licensing Enforcement does not accept anonymous complaints, and it sends a copy of the form to the person or business named.
